Form Submission Error & Mailchimp Issue

I am trying to test a form, but it does not seem to be working. I have followed all steps recommended on your website (From Address & SMTP Host [Google]) and I cannot figure it out.

I have also setup the mailchimp integration, but it does not seem to be working. I have used it before with no issues.

Also, regardless, I and the person filling out the form ARE receiving the information. I’d just like to have the page refresh to my homepage upon form completion (I have also tried the message option; did not work).
