FORM SUBMITS HALF

Hello,
I created a form with about 15 items.
When i finish to fulfill 5th line, form submits before finishing fulfilling. After refresh, visitors lose all data.
Is there any setting that i made mistake or how can i check this?

Secondly, even if i give background color to one cell, in frontside i cannot see changes.

I want to give a detail. This issue exist after checking radio buttons.
