Forms not always received and duplicates in backend

Hi,

We are having severe issues with the form. Sometimes the form submissions are being received via email and other times it is not. On a couple of occasions we have duplicates on the backend as well.

Its all very weird and has been a recent issue. Currently running as PHP.
