Formular don't show on the Page

Hi,

i generat a Formular but the problem is if i insert the shortcode from the formular in a page then don’t show the Formular. The Formular is acitvated.

If insert the shortcode for the fancy popup in the page then work the lightbox but i need the formular directly in the page.

pls help
