ParticipantAt first, I placed multiple upload forms onto my contact form and received some errors.
1) Can the script handle multiple upload functions placed throughout the contact form?

2) Then I deleted the other uploaders so that Ionly have ONE uploder on the page
Keep getting upload errors even though I created an upload folder AND inserted a path within the admin of the script.
Can you please give me an example of a how a FULL PATH should look?
And also any screenshots of any necessary from within the script’s admin panel

3) My current path looks something like…
/home2/hostusername/public_html/v3/wp-content/uploads/form_uploads/{form_id}/{year}/{month}/

4) Even when I uncheck “Save uploaded files to the server” I still get an “Upload error”.

I discovered the issue, BUT still need a fix or remedy.
Apparently, the uploader will only work if the permalinks are set to “Default”.
The issue is that I am using plugins that require my permalinks to be “CUSTOM” ( Day and name ) .
How do I get the uploader to work with a “custom” permalink ?
