How to display the form entries in the frontend?

I have create one form named “Sales Leads” to collect the purchase requirenet from registed user, And I wnat:

1. Registed User(Subscriber) can fill the form to submit the sales leads;
2. User can View/Edit theirself’s entries in the frontend;
3. Editor Role user can View/Edit all the entries of “Sales Leads” form;

Is is plugin support this feature and what need I to do?
