How to have unique web destination address for every submit with conditional log

Hello,
I just bought quform because of the conditional logic aspect and I wish to achieve the following. I’ve been working ON THE conditional logic for hours. When you visit http://www.edujay.com, you’ll see that I’ve done some conditional logic on side bar.

My question is: When my users click the final send button, I want them to go to a specific web address of the last option which I should supply. How can I ensure that every submit button links to a unique web address? Thanks in advance,
