hi thank you for the tip
1) where do I need to put this code/ file?
I tried to uploard file “quform-custom-code.php” to wp-content/plugins/quform/ folder and it’s not working.

2) also, want to show image in front of the list (image #1)

I also need to show uploaded image on the page and have the option to remove the image as well before submitting it.

Using the method above (uploading the script , i do not see the option to enable “Quform Custom Code” under the plugins area. after uploading the custom file into the plugins folder.
