Image Does Not Link To Form

I am unable to make an image popup in a Post for QuForm.

[iphorm_popup id=”3″ name=”xxxxxxx”]http://foreclosurepedia.org/wp-content/uploads/2014/06/xxxxx[/iphorm_popup]

This only creates a hyperlink to the form.
