Images instead of labels for checkboxes or dropdown

Hello,

I’m trying to ask a couple of question like: “Do you know who’s in this picture?” just by displaying someones’ picture and then I want to put some names in a dropdown for the visitor to choose from.

At the end I would like to be able to count the number of correct guesses.
Something like this: http://www.bemore.ro/formulare/Concurs.php

Can you help me with a few details ?
