1) – I am having issues where the checkboxes are running right off any mobile phone device that is held in portrait position.
I will send you a screenshot of what I mean.

2) – As a supplemental question, is there a way to just get the form to convert into just one column on mobile PHONES that are held in “portrait” position /
There a are some text fields where the form is set for 2 columns, but on a mobile phone that is held in “portrait” position , the text fields get squished.

And the text field for the Email Address gets squished.

BUT…
What I did was I made the Label Placement “Above” instead of “Left”.
So it now looks fine on portrait phones.
