Mail chimp integration

Hi,
I’ve installed Mailchimp Plugin to use Quforms forms to subscribe to join Mailchimp newsletter.
Now, no problem with installation. Api key verified.
Than I create a form in which there’re these fields:
-name (text)
-surname (text)
-email address (email)
-choose to which list do you want to be added (check filed) -> there are 2 option but you can select both
-pricacy check (that is a check with an option only)

The I create 2 integration related to list, with a logic option. One integration for one list, another one for the other list (2 in total).

I put the form in a page. Everything works fine, but no new contact is added to mailchimp list.
I paid attention to look in Mailchimp lists to find new contacts

So for the moment it doesn’t work. But I’ve some doubts about some points:
1) i set 2 fields: name and surname. How Mailchimp can understand that those values are name and surname valid for Mailchimp?
2) In Integration page, what is ‘Email address field’. It is autofilled with Email Address (31_5) and I can select just this. Is it ok?
3) I keep disabled both Merge and Double Out In
4) in the Groups field I can’t select because there’s no group to be selected

Could you please help me with that?

Hi,
I find sostino to my point 1 – name and surname, by using Merge.
So that point should be ok.

For the rest, still not working.
