Minimum Number of Entries & Email Verification on Voting Form

We have a voting form going live later this year, and I have posted the sample form https://healthlinkssc.com/quform-sample/ and would like to do 2 things:

1) Make sure that the submitter makes an entry in at least 5 fields in the “Vote for the Best!” section. How can I do this?

2) Verify the submitter’s email address. How can I do this?
