More complex "From" address

We would like to set up the “From” address so that it shows something like:

Web Inquiry From – Ben Welch, Miles of Chocolate

So the we have a text message of “Web Inquiry From – ” followed by the senders “email” and “company” values.

Is that possible?

When I replace the code, I am now getting this error:

Parse error: syntax error, unexpected ‘;’, expecting ‘)’ in /home3/dzlrmgoi/public_html/process.php on line 64

Also, we are trying to change the “Subject” so, does this go on line 70?
