my date element not looking on write place

Hi, i just create a small reservation form on wp side widget.
My date element is not on the write place and the pick calendar that i try to insert on my form does not work.

Hi again. I follow all the step of the plugin theme conflict but nothing change and i’m afaid that is my theme
