my form does not display normal

hi

I am using BETHEME with BEBUILDER …. I have installed the quform and watch all your videos in YouTube and start building forms. but as you see in the attached, the forms does not look normal. the form is not fullwidth and also I have build the multipage form but its showing all pages in single view ….

please advise
