Need assistance in creating a custom print layout

I have finalized my form but need help in creating a customized printing template/layout for the final completed form results. Please see attached photo of what I am trying to replicate.

Also how can I setup an email notification with the entry information displayed on a pdf? I only see options to export as an excel sheet.

Thank you for sending me this response. Upon submission, are there any functions to automatically send the entries’ excel file/csv via email notification instead of PDF? And if so, can i customize the layout?
