Not always recieiving notifications of form applicants

Although the data goes into the database and can be accessed in the back end the notifications only sometimes do not come through. This has happened to about 4 forms in 65 applicants? I have checked in case the person completing the form on the front end has used any weird symbols or something that may have confused the data base but nothing shows a pattern. Please can anyone help?
