Odd email format

In examining the headers for the email sent when a form is submitted, the FROM is built oddly. Is there a setting we’ve got wrong? It comes out this way:
From: “mark@” <pagemountain.com mark@pagemountain.com>
Reply-To: <mark@pagemountain.com>

Note, the Reply-To is correct, but the From is built incorrectly, repeating the domain and then giving the adddress. Oddly, the form is set to have a static From address which is info@kstransport.com. Here’s the complete header:
