Options Not Saving, Confused About Layout

For some reason the theme options (social media links, favicon, logo image, etc) are resetting and not saving. Tried disabling cache file, not sure what the problem is?

Also I see the layout for the logo in the middle, and I cannot figure out how to make that happen (see attached screenshot)

Any help is greatly appreciated – need site up asap!!
