Plugin insert [b] and [p] in the page

Hi

When I preview my form it looks OK, but once I insert it into a page it starts to add [b] and [p] (I used [] instead of HMTL tags). This makes the form look really odd and it adds 4-5 line breaks above the form – one for each hidden input element.

Please to assist.
