Popup and elements not working

Team,

Please take a look at the attached. I will appreciate your help.

1. The calendar on the date field does not show.
2. File upload does not show.
3. Even if i did not applied any css modifications to any of the groups the first group looks different.
4. The label does not disappear from inside of the box.
5. POP up does not point the form – instead points to #.
6. When combined with short-code displays “[raw]” link.
7. Firebug errors on the page
