Popup form not showing fields

Hi, I’m using a popup form for the first time
I think I follow all the instructions but when the form appears, no fields are shown (only HTML and checkbox options description). This is the page: https://www.labellaschool.it/corsi-audio-toscana/ableton-live-base/ (the form is shown by clicking on “Programma: CLICCA QUI”)

Debug.log doesn’t seem to show errors

Solved, the error was in the container!
