Popup Form Not Working

Hi guys;

I would like one of my forms to pop up when a guest clicks on a text link but nothing happens. I checked your troubleshooting and verified that my QuForm settings are correct. There are 2 syntax errors on the page but these seem to be in the QuForm code so I’m not sure how to proceed from here. Please see screenshot.
