popup link no longer working

I have been using this plugin with no issues until today. I updated my theme/wordpress and all of the sudden my popup forms no longer work. here is my info:

version. 1.4.11
wordpress 3.8
test URL: http://www.wevivefitness.com/test
I am using the following basic popup:
[raw][iphorm_popup id=”10″ name=”Combo – Gold”]Change this to the text or HTML that will trigger the popup[/iphorm_popup][/raw]

I have applied the fix suggested at:

Popup link does not work

and have Disable Fancybox output unchecked and Enable Fancybox is checked

please advise any more possible things to look into

fantastic troubleshooting and that fixed my problem in no time. jQuery issue is also fixed. Thanks again for the help Ally.
