Popup Via Button and Pass Parameters

I have four pricing tables, each for varying options (and price) for the same service. I want to have a form popup when the user clicks a button on one of the pricing tables. I also want the checkbox to be auto-completed based on whichever pricing option the user chose.

I can get the popup to work via a button. I can pass a parameter via a normal link to have the right checkbox be selected. What I can’t work out what to do is to combine the two.

Does anyone know how to achieve this?
