    I have a form defined with four fields (first_name, last_name, zip_code, & email_address). The form saves to a custom MySQL database. I enabled Prevent Duplicates for the email_address form, but I’m able to enter a record for the same email_address multiple times. I’m using QuForm for WordPress v. 1.7.7.

    Am I missing something? Everything I read said just to enable the option for the field you want to see if there is a duplicate.
