Problem of inactive form

Hello, i’ve put on my website 3 differents forms and one of these forms don’t allow me to write in fields ? But the active button in SETTINGS is ok. I don’t understand.

I’ve put the form in this code on the page.

I’ve try to put another form in place of this one and it’s works.
