QuForm Does not show up

Hey guys

.. after sitting there creating this quform with the nice backend panel, everything works in preview.

But when embedding the code on the page, Cuform does not show up. The code itself is there but it is set to visibility hidden – somewhere.
How can this be disabled?

Also when i disable this hidden thing in firebug temporary – the file-upload forms & buttons do not show up.

Any assistance would be helpful
