QUFORM EMail subscription questions

Hello,

I have created a banner that reads ‘click here to view an exclusive catalog’ I then want the user to click the banner and will be sent to a Quform where they fill in their email address and name. After they have done this they are shown a password or link to view or download the catalog I have created.

Can you tell me how i do this in Quform please
