Quform Formatting Issue

There is the issue I have been having. Imported the demo one, this is what I got.

It fixed, but now – the wrapping and CSS does not function right.
It’s the exact same as the demo form, so how do I fix the CSS?

I did import. It is the demo form, I just edited the fields.

I want it to look like the demo form. Please help me I payed $25 for this
