QuForm issue after installation – unable to add fields

I have successfully installed and verified license for Quform. I’m able to create a new form however most of the icons/options are not available! I can’t add simple text field. Basically I’m allowed to add only radio buttons. Can you please help me to trouble shoot the problem?

of course I’ve tried basic troubleshooting such as disabling all plugins and template. The issue still persists.
