Quform stopped working after transferring my site

For some reason after I transferred my site to a new server, the Quform stopped working. It works with other themes and it has worked on this theme in the past. The contact form can be seen on http://paveforce.com/contact . It just gets stuck on sending.

I suspect it could be a conflict with an old version of JQuery / Javascript or PHP. Thanks for your help!
