    Hi,

    I’m wondering If and if yes, how I can create a quiz with 25 questions, only yes/no answers, and depending on the answers 4 different end results. 1-3 times yes is a result, 4-7 times yes is a result, 8-15 times yes is a result and 16-25 times yes is a result.

    I would like to use a multi page form with 1 question per page.
    And probably redirect to separate pages showing the different results.
    But how do I keep score during this quiz?

    Would that be possible with Quform?

    Hi Ally,

    I think I’m missing something…

    I’ve added the form on this page:

    vitaliteitstest

    and these are the result pages:

    1 – 3 goed nieuws

    4 – 7 Tja

    8 – 15 Hmm

    16 – 25 Oei

    But after filling in the fields they all go to the url set to the default:

    16 – 25 Oei

    I can’t seem to change to order of the default to the last option.

    Another issue I encounter is the way the page loads between questions (form pages) is that something we can do something about?

    Hi Ally,
    Just to add some extra info: The hidden field collects the number of “yes” correctly. Attached a screenshot of the email with the fields.

    So I think it is related to the order of the confirmations..

    Hi Ally,
    Thank you for your feedback. I noticed the was no “is less than or equal to” option so I already added the Less than “one higher than the lowest option”.
    This unfortunately doesn’t work.

    I added admin login to the form in the footer

    As for the transition I think is was cache related or something, because it runs very smooth now. Please disregard the comment about the transition.

    You can disregards this question i figured it out and it works via the parameter in the redirect url.
    Quform is awesome!

    Hi Ally,

    I have a follow up question:
    I add a name field at on the last page of the test form.
    I would like this name to added to the forms that are located on the result page.
    I was thinking to add a hidden field in the form and adding dynamic value to it with a parameter name.
    But i’m not sure which parameter name should i add here to get this value send with the follow form?
