Quorum creates empty forms after editing

Suddenly in the last days when i edit a form or create a new one i get 2-3 empty ones created. See screenshot.

Thanks, tried to repair the table and this is the result:

Also tried to disable the plugin and reinstall it…same errors or issues.

Managed to do a workaround and created the form on an older installation with the 1.6 version and exported / imported the form.

Now it works and when i edit this version it saves it all correctly.
