Radio button messy size & spacing

Hello.

i found an issue on radio button:
1. i tried to customize 2 radio buttons. first one using “slim” button size, the other one using “fat” button size. both are appears unaligned. and also for medium size too.
2. the radio button shown an extra circle (on the left of the option)

i have tried to disable all plugins except default plugins for the theme i used.

please help me solve this issue. thank you in advance!
