Radio buttons conflicting with theme

Hi Ally, I have used radio buttons in my form but i believe there’s some CSS conflict and the radio/option buttons just appear as a line. Attached Form image has 2 locations, the line in front of both location is an option /radio button. Is there a way to work around this issue. Thanks

Looks like i also have some conflict with theme. radio buttons are missing and there is a shadow like dark circle if i place buttons. your help appreciated
