Some trouble with popup form

I am having a bit of trouble with the popup version of one of my forms. It seems that it is working fine and it opens in the popup when clicked once but if I close the popup and try to open the form popup again what opens in the page is a popup of the entire page, not the form.

Here is the code I am using:

<h5><i class=”fa fa-file-text”></i> [iphorm_popup id=”5″ name=”Locations Form”]Contact this Location[/iphorm_popup]</h5>
