Stripe integration

I’ve followed all the instructions on the Stripe Integration page but still don’t have it working.

At the top of the instructions, it shows a “card number” and “yy/mm” underneath the “credit card or debit*” field in the form. I have “credit card or debit*” but not the actual place to type in a card number.

What do I need to get this form working?
