Theme Not Working

I am having the same issues as several others in the forum but none of the solutions are working. It seems the theme in QuForm is completely ignored and the form uses the default theme of the WordPress template. I would like to use the “Simple” theme. How can I make that happen?

Thank you Ally. How can I change the button color and progress bar color from green to a custom? Everything else is working just fine.
