There was a problem. Not Found

Not sure why i get this message.
“The requested URL was not found on this server”
I created two forms, Contact form and Contact-Join form
The first on works no problem
The second returns the error message.
I figure it must be a php process thing but the first one works ok so I guess I’m stuck
