Two page contact form

I am looking to create a two-page contact form or something like this;
1. On The first page select the option the services and click on the contact link which leads to the contact us page.
2. Contact us page has a form but the option selected on the first page is already selected on the contact us page.

For eg: I have attached the screenshot. There is a widget (contact form 1) after selecting the services and clicking the continue, it takes you to the contact us page. On the contact us page the option selected in the widget is already selected.
