Unable to click form input / text area

I am unable to click in the form input field to enter my info. This issue has also been reported from my visitors.

My website is http://www.lindsaymarino.com It’s the opt-in form at the top in the blue bar. If you try to enter in your information the labels will not disapper so you can see and enter in your text.
