Upload button not visible

Hello,
I have created a pretty lengthy form and apparently everything seems to work but the file upload button.

It is not visible on the client side even though I have not touched any css of it. I am using a custom theme but even if i switch to no theme format the field does not show up

Firebug shows me css property has been set to hidden –

What do I need to do to enable upload button?

I am using Version 1.4.14
