Upload field required

I have a form located here: http://mig.cc/interstate-2014/
it will not let me upload a image when I press submit it keeps saying “this field is required” after I have selected an image to upload.
Please help.

Solved. I had to uncheck the “Enable Flash uploader”
This may be an update issue….
