User "Add/Expand" Rows

Is it possible to enable the end user to add/expand rows on the form. Something along the lines of this:

I have a form with the usual contact information, and then I’d like to collect a list of names. Initially I’d like the Names group to display two inputs but then give the user the option to add more fields.
