W3 Total Cache problems

Dear friends I have a problem when active w3Total Cache.
When show a full page size the form display OK.
But when resize
1.- The form lost dropdown list and required message styles.
2.- The form show some message and single box text.
3.- The form show message Please wait…

I review all the css code, and made the changes and work 100%
