ParticipantI didn’t do anything to fix the problem. I just remember looking at the form and it was not showing up. So I don’t know what the issue was, because a couple of hours later it was working fine.

Now its broken and the form doesn’t show up anymore… Did I do something wrong?

The website page is http://fusion-mag.com/nominate1/. The code is there, but its not showing up on the page.
