Getting 2 emails from form submission?

    Hi
    I have the form all installed and working, but i get 2 emails when the form is submitted?
    Im using two form options on the site. A contact form and a book an appointment form.
    From the contact form i get the emails with all the form details and then a second blank one? and from the appointments one i get the first one with the details and a second one with this info:
    First one;
    Positive Lending Appointments Contact Form
    Name Andy Seymour Esquire
    Telephone 07968582589
    Email address andy@complete-design.com
    Company The Design Corporation
    Location Ferndown national Park
    Loan Options Secured
    Mortgages

    Second one:
    name : Andy Seymour Esquire
    telephone : 07968582589
    email : andy@complete-design.com
    company : The Design Corporation
    location : Ferndown national Park
    loan_options :
    type_the_word : catch

    any ideas? is it something in the php process form? i have one for each form both with different names and the setting for that form.

    I dont know if this helps. the header info from the emails show this.
    The fully formatted email has this:
    From “Positive Lending” <appointments@positivelending.co.uk>
    To appointments@positivelending.co.uk
    Date Thu, 16 Apr 2015 14:34:59 +0100
    Subject Positive Lending Appointments Contact Form
    Client PHPMailer 5.2.9 (https://github.com/PHPMailer/PHPMailer/)

    The second non formatted emails shows this:
    From appointments@positivelending.co.uk
    To appointments@positivelending.co.uk
    Date Thu, 16 Apr 2015 14:34:59 +0100
    Subject Positive Lending Appointment Contact Form
    Client PHP/5.3.28
