Hi I have just tried to do a fresh install on a totally different site and the same problem occurs. It is a conflict between the new WordPress 3.5.1 update and this plugin.

Can you please advise on how to fix it?

This form is working, but when I try to create a new one the main plugin admin screen has no action and is totally blank.

Has anyone else had problems like this?

Hi Ally, thanks for the quick response. I have tried it but it seems to be not working. I am pretty sure it is what Yehudah has said and it is conflicting with the WP version.

If I do a database reset, does it wipe all of the info on there? as this will not be good.
